YLS Assessment List

WINDOW OVERVIEW:

The YLS Assessment List window opens when the YLS icon is selected and there are one or more assessments for the child.

The window is used to select the assessment that is needed.  To select the assessment highlight the assessment row and double click or highlight and select the icon.

FIELD DESCRIPTIONS:

ARP ID:  This field displays the random/unique N-FOCUS number assigned to the agency related person  It is for display purposes only

Name:  This field displays first and last name of the Juvenile being assessed.

Assessment List Box:

Assessment Date:  This column displays the date selected by user to best indicate when the assessment was completed or started.

Status:  This column displays the status of the assessment.

Risk Level:  This column displays the summation score of all the characteristics chosen.

Status Begin:  This column displays the date the status was set.

Assigned To:  This column displays the staff person responsible for the assessment.

Approved By:  This column displays the staff person that approved the assessment when is has been approved.

Assessment Nbr:  This column displays the random/unique N-FOCUS number assigned to the assessment.
