Detail Service Needs Assessment
WINDOW OVERVIEW:
The Detail Service Needs Assessment window is used to create new service needs assessments and display the details of service needs assessments.
The assessment tool automates and standardizes the client assessment process. It is used to identify the tasks requiring assistance by the client to perform daily activities and documents the time required to complete each task.
The Detail Service Needs Assessment window is opened from the SNA icon on the N-FOCUS Main Menu or the Person Detail window; from opening a highlighted assessment line on the List Service Needs Assessment window or from selecting the New button on the Search Service Needs Assessment window or List Service Needs Assessment window.

FIELD DESCRIPTIONS:
Assessment Group Box:
Number: This field is blank for a new assessment and displays the unique system generated ID number for the assessment for established assessments.
Recommendation: This field is blank prior to an approval of the assessment. When the approval is made this field will display the name of the program recommended to pay for the services.
Mode: The current window mode displays under the close icon.
Person Group Box:
Name:
This field
is blank for a new assessment when the window is opened from the SNA icon
on the N-FOCUS Main Menu or from the Search
Service Needs Assessment window. Select
the black arrow to open the Person Search
window. Enter
the name of the person for whom you are searching and if they are found
on the Person List window, highlight the person's
name and use the blue return arrow
to bring the name back
to this window. If
only one person with the name is found the name will automatically return
to this window.
If the window is opened from the Person Detail window, from opening a highlighted assessment line on the List Service Needs Assessment window, or from selecting the New button on the List Service Needs Assessment window the field will be populated with the person's name.
Number: This field is completed with the unique system assigned ARP number.
Begin Date: Enter the date (MM-DD-YYYY) the client and worker complete the assessment.
- This date may be changed to a past date if the assessment is in pending status. A message may display if an incorrect date is entered.
- This date cannot be a future date.
End Date: When the assessment is approved, the system sets the end date field to the last day of the 12th month from the begin date.
Status: This field displays the status of the assessment.
Answer and Question List Box:
Using the Yes and No buttons below, highlight and answer each of the questions.
- All questions must be answered.
- If the client is potentially eligible for continuation of assessment based on the answers, the Service Needs button enables.

The Service Needs button is enabled when the answers to the assessment questions determine the person is potentially eligible for services. Select the button to open the Service Needs Time Assessment window. |
