Service Needs Assessment Status Summary

WINDOW OVERVIEW:

The Service Needs Assessment Status Summary window is used to view a summary of the assessment.  

This is a view only window.

FIELD DESCRIPTIONS:

Status:  This field displays the status
DRAFT
DENIED
APPROVED of the assessment

Recommendation:  For approved assessments this field displays the recommended program.

Status Reason:  For denied assessments this field displays the reason the assessment was denied.

Completed by:  This field displays the name of the worker who made the assessment determination.

ACTION BUTTONS:

	Select the Central Office Approval button to view the Update Service Needs Assessment Question window with the central office approval question.
	Select the Central Office Approval button to view the Update Service Needs Assessment Question window with the Physician/RN question.
	Select the Central Office Approval button to view the Update Service Needs Assessment Question window with the provider availability question.
	The OK button is not enabled.
	Select the Cancel button to close the window and return to the previous window.
