Update Service Needs Assessment Question

WINDOW OVERVIEW:

The Update Service Needs Assessment Question window is a set of three questions that further define the assessment of the service need.  The questions can be completed from the Detail Service Needs Assessment Actions drop down menu or viewed from the Service Needs Assessment Status Summary window.  The Provider question also opens from the Approve Service Needs Assessment window.

FIELD DESCRIPTIONS:

Selection Box:

No:  Select the No radio button if the answer to the question is negative.

Yes:  Select the Yes radio button if the answer to the question is positive.

Questions:

HAS CENTRAL OFFICE APPROVAL BEEN GIVEN?:  This question refers to Central office approval for more than 40 hours/week of PASS/PAS services only.

IS THERE A PROVIDER IDENTIFIED TO PROVIDE THE CLIENT WITH SERVICES?:  This question refers to the availability of a service provider.

HAS A PHYSICIAN/RN SIGNED STATEMENT BEEN RECEIVED?:  This statement refers to the statement needed for Specialized Services.

ACTION BUTTONS:

	Select the OK button to save the Yes or No answer selection to the question and return to the previous window.
	Select the Cancel button to close the window and return to the previous window without saving a selection.
