Name History
WINDOW OVERVIEW:
The Name History is an inquiry window that displays the name history of a person. It is automatically updated by the system each time there is a change to a person’s name on the Person Detail window. The worker may also update the name history from this window to enter previous names the person has been known by. The name of the worker that created and updated the person information is displayed.

FIELD DESCRIPTIONS:
Name History List Box:
Last:This field displays the last name of the person as of the create timestamp date.
First: This field displays the first name of the person as of the create timestamp date.
Middle: This field displays the middle name or initial of the person as of the create timestamp date.
Ext.: This field displays the name extension of the person as of the create timestamp date. Example: Jr., IV
Type: This field displays whether the name history was created by the system or a worker. System generated entries will have “system”. Worker generated entries will have “AKA” (also known as) or “Maiden” (maiden name).
Begin Date: This field displays the date the name history was created.
End Date: This field displays the date a former name was ended.
Updated: This field displays the user ID of the staff person making the change.
Person Created By Group Box:
Create UserID: This field displays the User ID of the staff person who created the ARP.
Create Timestamp: This field displays the date the staff person created the ARP.
