The FCRO Services to Child window is used to document information collected during the review of a child in care.. It includes information specific to the child.
This window can be opened from either the Detail FCRO Review window or the List Detail FCRO In Care window
Fields with an asterisk * are a reminder to FCRO staff that these are not historical fields.
Where codes are used, the explanation of the code displays when the window is saved. If an incorrect code is entered the field will turn red and must be corrected.

Document FCRO Services to Child Information
Last: This field displays the last name of the FCRO child. For private adoptions, this field displays the adoption identifier.
First: This field displays the first name of the FCRO child. For private adoptions, this field is blank.
Mode: The current window mode displays under the Close icon.
Reasons Entered Care*: Enter up to 12 codes for the reason(s) the child entered into care.
Later Disclosed Conditions: Enter up to 12 codes for reasons that were disclosed at a later date.
Physical Exam Date*: Enter the date (MM-DD-YYYY) of the physical exam.
Physical Exam within 2 weeks*: Enter the code to indicate if the physical exam was within two weeks.
Services to Child: Enter up to 6 services to the child codes.
Disabilities Diagnosed*: Enter the code to indicate if disabilities have been diagnosed.
Child's Disabilities*: Enter up to 6 disability codes for the child.
Health Record in File: Enter the code to indicate if a health record for the child is in the file.
Education Record in File: Enter the code to indicate if an education record for the child is in the file.
Enrolled In Special Education: Enter the code to indicate if the child is enrolled in special education.
Applied for ADD Service*: Enter the code to indicate if ADD service has been applied for the child.
History of Self Injury: Enter the code to indicate if the child has a history of self injury.
Injury to Others: Enter the code to indicate if the child has a history of injury to others.
Sexually Acting Out: Enter the code to indicate if the child is sexually acting out.
Alleged Sexual Perpetrator: Enter the code to indicate if the child is an alleged sexual perpetrator.
Number of Disclosed Victims: Enter the number of disclosed victims.
Contracted Services: Enter the contracted services code.
Service Contractor: Type in the name of the agency contracted to provide case manager or placement services.
Child/Worker Contact: Enter the code to indicate if there was contact with the child's case manager.
Child/GAL Contact: Enter the code to indicate if there was contact with the child's GAL.
