WINDOW OVERVIEW:

The Application Details window for reopened program cases is used to record only the date the case is reopened.

The Application Details or Program Case Application Dates for new program cases window is used to record the Application Request Date and Application Received Date for new program cases.

Add/Reopen Program Case

FIELD DESCRIPTIONS:

Reopen Date: The date field for reopened cases will be blank. Enter the date the case is to be reopened.

OR

The dates entered in these two fields may or may not match depending upon the case circumstances. Both dates will default to the current date.

Request Date: Enter the date the family requested this assistance.

Received Date: Enter the date the Department received the signed application for assistance.

ACTION BUTTONS:

	Select the OK button to close the window and save the changes.
	Select the Cancel button to close the window without saving any changes.
	Select the Help button to display window specific Help text.
