Program Case Begin Date

WINDOW OVERVIEW:

The Program Case Begin Date window is used to record the date the program case is opened for the following programs:

• Developmental Disabilities Service Coordination
• Developmental Disabilities State Aid
• Former Ward (window title changed to Program Case Eligibility Date)
• Juvenile Court
• Independent Living (window title changed to Independent Living Begin Date)

How Do I

Add/Reopen Program Case

FIELD DESCRIPTIONS:

Begin Date: Enter the date the program case began.

ACTION BUTTONS:

	Select the OK button to close the window and save the changes.
	Select the Cancel button to close the window without saving any changes.
