WINDOW OVERVIEW:

The Program Information window is used to document application information for the following programs:

• Adult Protective Services
• Emergency Assistance
• Part A Beneficiary
• Public Institutions Medicaid
• Social Services Aged/Disabled
• Social Services Children and Family
• Subsidized Adoption Medicaid Only
• Subsidized Adoption Payment Only
• Subsidized Adoption/Medicaid
• Subsidized Guardianship Medicaid Only
• Subsidized Guardianship Payment Only
• Subsidized Guardianship/Medicaid

FIELD DESCRIPTIONS:

Benefit Start Date: Enter the date the program benefits began.

Application Request Date: Enter the date the client requested benefits.

Application Received Date: Enter the date the application was received in the local office.

Assistance: Select the type of assistance from those in the drop down list.

Fund Code: Select the type of fund code from those in the drop down list. Not all Assistance types require a fund code.

ACTION BUTTONS:

	Select the OK button to close the window and save the changes.
	Select the Cancel button to close the window without saving any changes.
