WINDOW OVERVIEW:

The Waiver Assessment Date window is used to enter the date the person meets the criteria for the specific Waiver program.

FIELD DESCRIPTIONS:

Date: Enter the date the person meets the criteria for the listed Waiver program.

ACTION BUTTONS:

	Select the OK button to save the date information to open the waiver case.
	Select the Cancel button to close the window without saving the date or opening the waiver case.
	Select the Help button to display window specific Help information.
