Search Service Needs Assessment

WINDOW OVERVIEW:

The Search Service Needs Assessment window is used to enter the criteria to search for a service needs assessment or to create a new service needs assessment.  The window is opened from the Service Needs Assessment icon on the N-FOCUS Main Menu.

Search for a Service Needs Assessment

FIELD DESCRIPTIONS:

Assessment Group Box:

Number:  Enter the Personal Assistance Service (PAS) Assessment Number if known.

OR

Person Group Box:

Name:  Select the black arrow to open the Person Search window.  Enter the name of the person for whom you are searching and if they are found on the Person List window, highlight the person's name and use the blue return arrow to bring the name back to this window.  If only one person with the name is found the name will automatically return to this window.

SSN:  Enter the person's Social Security Number if known.

ACTION BUTTONS

 The Search button will be enabled when an assessment number, person name, or SSN is entered.  Select the button to begin the search process.  The List Service Needs Assessment or Detail Services Needs Assessment window will open for the selected person.  If there is no match for the search criteria, a message displays.
 The New button is enabled when there is no search criteria entered on the window.  It is used to open a blank Detail Service Needs Assessment where a new assessment can be created.
 The Clear button removes any search criteria from the window so that new criteria can be entered or an new assessment created.
 The Cancel button closed the window and returns to the N-FOCUS Main Menu.
 The Help button opens the help instructions for the active window.
