FCRO Recommendation Report

The FCRO Recommendation Report window is used to document information regarding the review board meeting and the recommendations.. Information includes barriers to permanency, the board’s findings, the name of the judge, GAL, tribal judge, CASA worker, and the child’s attorney.  Information may include to whom the questionnaires were sent, who returned them, who was invited to the board meeting and who attended, and to whom the recommendation report was sent.   .

This window can be opened from either the Detail FCRO Review window or the List - Detail FCRO Injury window.

Fields with an asterisk * are a reminder to FCRO staff that these are not historical fields.

There are no mandatory fields.  Any of the other fields may be updated or left blank.

Where codes are used, the explanation of the code displays when window is saved  If an incorrect code is entered the field will turn red and must be corrected.

FIELD DESCRIPTIONS:

Last: This field displays the last name of the FCRO child. For private adoptions, this field displays the adoption identifier.

First: This field displays the first name of the FCRO child. For private adoptions, this field is blank.

Mode: The current window mode displays under the Close icon.

Judge*:  Enter the name of the judge.

GAL*:  Enter the name of the GAL.

CASA*:  Enter the name of the CASA worker.

Child's Attorney*:  Enter the name of the child's attorney.

Tribal Judge*:  Enter the name of the tribal judge.

Number Invited:  Enter the number of people invited to the review meeting.

Number Participated:  Enter the number of people who participated in the review meeting.

Review Participants

Court and Judge:  Enter the appropriate code in each column to indicate if the court and judge was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

County Attorney:  Enter the appropriate code in each column to indicate if the county attorney was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

GAL:  Enter the appropriate code in each column to indicate if the GAL was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Custody Agency:  Enter the appropriate code in each column to indicate if the custody agency was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Parent's Attorney:  Enter the appropriate code in each column to indicate if the parent's attorney was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Second Parent's Attorney:  Enter the appropriate code in each column to indicate if the second parent's attorney was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Child's Attorney:  Enter the appropriate code in each column to indicate if the child's attorney was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Parole Prob. Officer:  Enter the appropriate code in each column to indicate if the parole or probation officer was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

CASA:  Enter the appropriate code in each column to indicate if the CASA worker was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Mom's GAL:  Enter the appropriate code in each column to indicate if the mother's GAL was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Dad's GAL:  Enter the appropriate code in each column to indicate if the father's GAL was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Legal Party 1:  Enter the appropriate code in each column to indicate if another legal party was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Legal Party 2:  Enter the appropriate code in each column to indicate if a second legal party was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Mom:  Enter the appropriate code in each column to indicate if the mother was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Dad:  Enter the appropriate code in each column to indicate if the father was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Foster Child:  Enter the appropriate code in each column to indicate if the foster child was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Foster Parent:  Enter the appropriate code in each column to indicate if the foster parent was sent a questionnaire, returned the questionnaire, was invited to the meeting, attended the meeting, and was sent a recommendation report.

Barriers

Enter up to 10 of barrier codes.  The barriers are listed in 9 categories  Not all categories will have barriers and some categories may have more than one barrier.
