List - Detail FCRO Injury

The List - Detail FCRO Injury window is used to document information Injuries to a child during the review period.  Information includes type of injury site, if the injury was the result of restraint, type of injury, service provider where injury occurred, and whether the provider is still in use.

This window can be opened from either the Detail FCRO Review window or the List - Detail FCRO Restraint window.

The Type of Injury site and Type of injury are the only mandatory fields.  Any of the other fields may be updated or left blank.

Where codes are used, the explanation of the code displays when the Add button is used to move the information to the list box.  If an incorrect code is entered the field will turn red and must be corrected.

If the child was restrained during care of a service provider, that information is documented by adding the name of the provider organization.

FIELD DESCRIPTIONS:

Last: This field displays the last name of the FCRO child. For private adoptions, this field displays the adoption identifier.

First: This field displays the first name of the FCRO child. For private adoptions, this field is blank.

Mode: The current window mode displays under the Close icon.

Injury List Box:

The Injury list displays a list of all incidents of injury associated with this review.  The list will be sorted by type of injury site.  The details of an injury can be updated by highlighting a line, making the change(s), and selecting the Update button.  

Injury Site:  This column displays the type of injury site.

Injury Type:  This column displays the type of injury.

Restraint:  This column displays if restraint contributed to the injury.

Service Provider:  This column displays the Service Provider if injury occurred in their care.

Injury Group Box:

Type of Injury Site:  Enter the type of injury site code.

Was Injury due to Restraint:  Enter the injury due to restraint code

Injury Type:  Enter up to 4 injury type codes.

Service Provider Where Injury Occurred:  Enter the N-FOCUS Organization ID Number.  If the service provider is unknown leave this field blank.

 Select this arrow if the Organization ID is unknown.  The Search Organization window opens to begin the search for the appropriate provider number.
 The Organization icon is enabled when an organization has been selected.  Use the icon to open the Detail Organization window .

Name:  The name of the selected organization displays in this field.

Provider Still in Use:  Enter the code to indicate if the provider is still in use.

ACTION BUTTONS:

 The Add button is enabled when the type of Injury site and type of injury fields are completed.  Select this button to move the information from the injury group box into the injury list box.

 The Update button is enabled when a line is highlighted in the injury list box.  The information from that line is also displayed in the injury site, injury type, restraint, and service provider fields.   The information in these fields can be updated and the update button selected to change the information in the list box.
 The Remove button is enabled when a line is highlighted in the injury list box.  Select the button to remove the information in the selected line.  A warning message will appear to confirm the remove action.  This can be used to correct errors such as the wrong organization selected.
