Service Request
WINDOW OVERVIEW:
The Service Request window opens when the Service Request icon is selected from the Detail Program Case window or the Person Detail window (only
available for existing requests).
This window is used to document persons waiting to receive Developmental Disabilities services. The information is used to track funding and justify requests for addition funding.
This window is inquiry only for everyone except Central Office Development Disabilities staff.
The Service Request window tracks:
- Wait for funding – persons who have met the criteria for Developmental Disabilities but have not yet been given priority for funding
- Wait for a provider – persons who have met the Developmental Disabilities and have been given priority for funding, but do not have a provider for the service.
- Service interrupted – persons who have had their services stopped (interrupted)for an extended period of time.
Developmental Disabilities Overview
FIELD DESCRIPTIONS:
Person Name: This field displays the name of person who is requesting services.
Service List Box:
The Service list box displays the service request(s). The box is blank on new requests until the information in the Service Request group box has been added.
Service Name: This field displays the name of the requested service.
Req. End Dt: This field displays the date the service request ended. Open requests will display above the closed requests.
Req. Date: This field displays the date the service was requested.
Date Wanted: This field displays the date the person would like the service to begin.
Type: This field displays the type of service request.
Provider: This field displays the name of the service provider.
Location: This field displays the preferred DD Service Coordination office location.
Service Area: This field displays the general Service Area where the client would like to receive services.
Reason Ended: This field displays the reason a service request has ended.
Funding Source: This field displays the source of specialized funding for the service request.
Funding Amount: This field displays the amount of money allocated to meet the service request.
Last Update Dt: This field displays the date the service request was last updated.
Service Request Group Box:
The service request group box is used to add or update the details of the service request. The service, original request date, and type are mandatory. Either a Preferred Provider, Preferred Service Area, or Preferred Location must also be selected. To add a new request, complete the fields as appropriate, and select the add button to move the information to the Service list box. To update an existing request, highlight the request in the Service list box and the current information will display in the Service Request group box. Make any changes and select the "update" button to move the information to the Service list box.
Service:
This field displays the ID number of the requested Service Type. To change
the Service Type enter the new service type ID number. If not known, use
the search arrow or select the "Select Service"
option in the Actions drop down menu to open the Search Service Type window to search for the
type of service requested. The name of the Service Type will appear below
the ID number.
Original Request Date: Enter the date the service was requested.
Date Wanted: Enter the date the person would like the service to begin.
Type: Select the type of request from the drop down list.
Preferred Provider: This field displays the ID number of the requested
provider. To change the Preferred Provider enter the new provider ID number.
If not known, use the search arrow, or select the "Select
Provider" option in the Actions drop down menu to open the Search
Organization window
to search for the requested provider. The name of the Preferred Provider
will appear below the provider ID number.
Preferred Location: This field displays the ID number of the requested
DD Service Coordination office location. To change the Preferred Location
enter the new office ID number. If not known, use the search
arrow, or select the "Select Office" option in the Actions drop
down menu to open the Search Office
window to search for the requested office location. The function selection
on the Search Office window will default to the DD Service Coordination
selection. On the Service Request window the name of the Preferred Location
will appear below the office location ID number.
Preferred Service Area: Select the general service area from the drop down list.
Request End Date: Enter the date the request for service ends. The Reason Ended field must also be completed.
Reason Ended: Select the reason the request has ended from the drop down list.
Special Funding: Select the source of any specialized funding from the drop down list.
Funding Amount: Enter the amount of money allocated to meet the request.
ACTION BUTTONS:
These actions can also be completed from the Actions drop down list.

Select the Add button to add the information entered in the Service Request group box. The added request then appears in the Service list box. |

The Update button is enabled when a request is highlighted in the Service list box. Select the button to move the updated information in the Service Request group box to the Service List box. |
