Enter Professional Relationships
Case Person: The Case Person for whom the Medical Exam information is displayed.
Appointment Date: The Date of the Medical Exam Type selected from the Type field.
Type: Select the type of Medical Exam from the drop down. (Dental, Physical, Psychological or Vision)
Follow-up Appt Date: The date of the next follow up appointment.
Professional: The name of the professional who conducted the selected medical exam.
Note: Select the Out Select Arrow icon to search for the Professional who is conducting the medical exam.

Click to add the Medical Exam information entered. |

Select this button to update a selected Medical Exam from the group box at the bottom of the window. |

Medical Exam Group Box Details
The group box at the bottom of the window shows a list of Medical Exams for the Case Person.
Date: The date of the Medical Exam.
Type: The type of Medical Exam is displayed.
Follow Up Appt Dt: The date of the next follow up appointment.
Professional: The Professional who conducted the Medical Exam.
Role: The Role of the Professional who conducted the Medical Exam.
