YLS Assessment Detail

WINDOW OVERVIEW:

The YLS Assessment Detail window is used to view an assessment or create an assessment. The window opens when an assessment is selected on the YLS Assessment List or when the YLS icon is selected on the Person Detail window and no assessment has been created.

FIELD DESCRIPTIONS:

ARP ID: This field displays the random/unique N-FOCUS number assigned to the agency related person It is for display purposes only.

Name: This field displays first and last name of the Juvenile being assessed.

Status: This field displays the Status of the Assessment.

Assessment Date: This field displays the current date if this is a new assessment but it can be changed to an earlier date (MM-DD-YYYY). For assessments already crated and saved this is the date entered by the user.

Documented By: This field displays the name of the person initially entering the information but may be changed by selecting the arrow to open the Search Staff Person window. The name is this field should reflect the worker who actually conducted the interview or documented the assessment.

Assessment Type: From the drop down list, select the Assessment Type that describes the Juveniles situation at the time the Assessment was conducted.

Domain List Box:

There are eight domains for YLS assessments. Each domain has its own characteristic detail window for scoring purposes. To open the YLS Domain Characteristic Detail window for the specific domain double click the domain name. Only one domain can be selected at a time.

Domain: This column displays the name of the Domain.

Score: This column displays the score for each Domain if the domain has been completed and scored.

Risk Level: This column displays the risk level for each Domain.

Strength: This column displays the strength indicator if the selected domain is a strength for the juvenile.

Total Score: This field displays the total count for all of the selected Characteristics.

Risk Level: This field displays the total risk level for the assessment as determined by the total count of the selected Characteristics.
